Who is Eligible
Who it is for?
The Breast Prostheses Subsidy is a Government funded payment to people who have undergone a partial or full mastectomy, either unilateral or bilateral, and/or have undergone a lumpectomy, have congenital needs, or have had reconstructive surgery, as specified by a specialist or general practitioner (GP).
Who can claim?
You can claim for this service payment if you: are a New Zealand citizen, or are ordinarily resident in New Zealand; and your specialist or GP gives you a current medical certificate with your NHI number, and confirms your need for breast prostheses services.
How much can you claim?
Your entitlement is $613.33 (incl GST) per side for a 4 year period. The entitlement period begins the day you make your first purchase and ends 4 years from this date. In certain circumstances this will continue to roll over.
How to use your claim through us
Claiming yourself
Place your order and pay for your products online. A copy of your purchase will be emailed to you. We will send your products as soon as they are available. To make a claim for reimbursement you must first have a Sector Operations payee number. Ring 0800 855 066 – (option 2) for a "Payee Number Application Form" and a "Breast Prostheses Claim Form", or use the links below. Complete the forms and return them to Sector Operations for reimbursement of your costs.
We claim on your behalf
When placing your order, tick "add notes to your order" and state that you wish to use your subsidy. Download the "Breast Prostheses Claim Form" below, print and compete the form, then upload via our "Contact Us" page. If it is your first claim, you will also need to upload your medical certificate. Once the paperwork has been correctly received, we will send your products as soon as they are available. You do not pay for the products yourself, we get paid directly from Health NZ.
At the Checkout
Before checking out, make sure you have downloaded your Claim Form and filled it out.
At the checkout, choose Subsidy Claim under payment method, upload your claim form to us on our contact page and proceed to checkout.
Once we have received your order and claim form we will process your order and send it off for delivery.
